Request your Kratom COAs through your WAAVE dashboard

*In your WAAVE Dashboard, go to Compliance - Order COAS
*In Pay for COAs - Select Product Type, select Cannabinoids or Kratom - Alkaloid Potency and hit the NEXT button
*On this screen, you can either Pay with your primary card OR Update your primary payment method. Please, read the information carefully.
*After hitting the Pay with your primary card button, you will see the following screen where you will need to complete the form below. You can request three product COAs at a time; if you need more COAs please start a new order. Hit the Next button and follow the instructions. Please, be really careful when completing the forms.
